At the National Center for Medical Home Implementation, many types of assistance are available:

General Technical Assistance
Technical assistance is offered to anyone who is involved in ensuring all children and youth have the services and support to access to a medical home. Examples of technical assistance are:

  • Identification of barriers and assistance in overcoming them
  • Strategic planning to build systems of care to support medical homes
  • Identification of local, state, and national resources
  • Linking individuals to the medical home contacts in their states
  • Updating medical home state webpages
  • Identification of funding sources
  • Dissemination of resources from the National Center for Medical Home Implementation
  • Developing and disseminating medical home tools for families, providers, youth, insurers, communities and states

Screening & Surveillance Technical Assistance
Pediatricians and other health care professionals who are interested in integrating screening and surveillance activities with the medical home can download tools and resources on newborn and infant hearing screening, newborn metabolic / genetic screening, preschool vision screening, and developmental screening and surveillance. National Center staff are available to provide assistance in any area and in facilitating the development of strategies throughout the process.
