There are many resources available for CSHCN living in Alameda County; however,
understanding the various service systems providing support can be a daunting task.
Each program has its own regulations, eligibility requirements and funding streams.
The goal of the Alameda County Medical Home Project for CSHCN Resource Guide is to
aid providers and their staff in making appropriate referrals by providing a
“snapshot” of each program, including:
