Children’s Special Health Services (CSHS) of the North Dakota Department of Health:

  • Distributes information on medical homes to increase public awareness and facilitate practice implementation.
  • Collaborates with partners to further the medical home concept and practice in North Dakota (e.g. Support completion of Integrated Services Grant activities).
  • For children receiving CSHS coordination services, monitors the medical home status and the percentage of children with a service plan.
  • Seeks out sources of funding to support implementation of medical homes and provide grant funding to support medical home infrastructure development in ND (e.g. submit optional legislative budget request/provide contract funding to the ND Chapter of AAP, etc).
  • Obtains training, technical assistance, and/or mentoring support to further knowledge of medical homes in practice.

At this time, pediatric practices are being recruited and learning collaboratives are underway. Some pilot projects are conducting activities throughout the state. For more information, contact:

The North Dakota Integrated Services Project
The North Dakota Integrated Services (NDIS) project is a three-year grant funded by the US Department of Health and Human Services – Maternal and Child Health Bureau to the North Dakota Center for Persons with Disabilities at Minot State University. The NDIS project will assist state agencies, local medical providers, and families of children and youth with special health care needs, ages birth to 21, to develop the knowledge and infrastructure to assure that all children and youth receive the coordinated care they need. Medical Home Liaison Grant to the North Dakota AAP Chapter
The American Academy of Pediatrics – North Dakota chapter received a grant from Children’s Special Health Services of the North Dakota Department of Health for a Medical Home Liaison. This position is responsible for seeking funding sources for care coordination activities, assisting with website development, and serving as a link between the chapter and various collaborative activities. Title V Block Grant to States
Title V of the Social Security Act is one of the largest Federal block grant programs. It leads the nation in ensuring the health of all mothers, infants, children, adolescents, and children with special health care needs (CSHCN). Title V is administered by the Maternal and Child Health Bureau (MCHB) as part of the Health Resources and Services Administration, U.S. Department of Health and Human Services.

MCHB Objective: Support development and implementation of comprehensive, culturally competent, coordinated systems of care for the estimated 18 million U.S. children who have or are at risk for chronic physical, developmental, behavioral or emotional conditions and who also require health and related services of a type or amount beyond that required by children generally.

Family Voices of North Dakota
Family Voices of North Dakoata assisst families as they navigate public and private systems, including health systems and insurance plans and provide assistance to families in accessing services and resources for their children and how to partner with providers and caregiver.

North Dakota FamNet
ND FamNet is a collaborative rural health network that specializes in providing family support information and resources, currently housed at the ND Center for Persons with Disabilities at Minot State University. ND FamNet provides information and referral to a variety of rural family support options, a collaborative training/event calendar, a collaborative training library and assistance in supporting challenged family populations.

Early Hearing Detection & Intervention (EHDI) Contact(s):
State EHDI programs promote universal newborn hearing screening, develop effective tracking and follow-up as a part of the public health system, promote appropriate and timely diagnosis of the hearing loss, prompt enrollment in appropriate Early Intervention, link newborns to a medical home and strive to eliminate geographic and financial barriers to service areas.

State Waiver Information
Waivers are the result of a process that allows state Medicaid agencies to apply for and receive permission from HCFA to provide services not otherwise covered by Medicaid and/or to do so in ways not described by the Social Security Act. Most Medicaid managed care programs require Waivers. The Waivers, which can differ greatly, are known by their numbers (1115, 1119), or as home-and community-based, or as Katie Beckett Waivers.

Autism Service Guidelines
Guidelines: Identifying, Serving, and Educating Children and Youth with Autism
The document (published in 2003) provides practical and useful guidelines based on a sophisticated and rigorous analysis of the needs of young children. It is intended to be a valuable resource to help make informed decisions with respect to assessment and intervention for young children with autism.

Great Plains Autism Program
The Great Plains Autism Spectrum Disorder Treatment Program provides training, research, diagnostic and treatment services to North Dakota children and youth suspected or diagnosed with Autism Spectrum Disorders (ASD).
