Objectives: By the end of this teleconference the participants will be able to:
1. Learn about and gain access to essential tools to have in practice that provide a successful medical home for children and youth.
2. Understand how those tools and guidelines can lead your practice towards passing Level 1 of the National Committee on Quality Assurance (NCQA) standards of care.
