Objectives: By the end of this teleconference the participants will be able to:

1. Discuss the importance of primary and specialty care collaboration in the provision of comprehensive, family-centered care provided in the medical home.
2. Describe the benefits of enhancing communication in the continuity between health systems.
3. Identify 3 practical approaches to enhance communication and co-management and their potential applications for improved chronic condition management in their practice.
