Webinar: How to Enhance Care Delivery for a Diverse Population
Co-sponsored by the National Center for Medical Home Implementation and the National Center for Cultural Competence
Understanding cultural issues is an important component of providing quality health care in the medical home as the US ethnic and racial make-up is changing. Faculty for this webinar will discuss how to provide culturally competent care to patients and families, especially those with limited English proficiency or from immigrant families. Promising practices in the delivery of language access services will also be highlighted.

April 7-9, 2013—Scottsdale, AZ
14th Annual International Summit on Improving Patient Care in the Office Practice & the Community
The 14th Annual International Summit, hosted by the Institute for Healthcare Improvement, will include 12 highly interactive, full-day session pre-conference minicourses on Sunday, April 7, 2013. The two-day general conference on April 8 and April 9 feature keynotes, special interest keynotes,rapid-fire workshops, and a wide selection of educational sessions. The International Summit will also feature quality improvement storyboards, an educational exhibit area, and numerous opportunities for networking. Session details will be available Fall 2012.

12th Annual Early Hearing Detection and Intervention (EHDI) Meeting
This meeting brings together a wide variety of attendees including those who: work in state EHDI programs; assist in EHDI efforts on the federal level; provide screening, diagnostic and early intervention support at the state/local level to young children with hearing loss and their families; champion medical home activities within each state; are parents of children with hearing loss; or are deaf or hard-of-hearing adults who are helping to expand opportunities for young children with hearing loss. The goal of the meeting is to enhance the implementation of comprehensive state-based EHDI programs. After attending the meeting, participants will be better able to identify and promote knowledge and awareness of successful strategies for implementing comprehensive state-based EHDI programs that include screening, audiologic diagnostic evaluation, early intervention, medical home, reporting, tracking, surveillance, loss to follow-up issues, data integration, and other related surveillance components.

Moving Forward with Patient- and Family-Centered Care—Partnerships for Quality and Safety
This Institute for Patient- and Family-Centered Care seminar provides three and a half days of comprehensive and practical sessions designed to help administrative leaders, board members, physicians, nurses, staff, and patients and families become effective agents for patient- and family-centered change in their organizations. Participants will gain the knowledge and skills to begin to transform health care within hospitals, ambulatory care settings, and community practices to address the challenges and recommendations discussed in the Institute of Medicine’s report, Crossing the Quality Chasm: A New Health System for the 21st Century and more recently, the National Priorities Partners report, National Goals and Priorities: Aligning Our Efforts to Transform America’s Healthcare.

5th International Meeting on Indigenous Child Health (IMICH)
 Join the Canadian Paediatric Society and the American Academy of Pediatrics for the 5th IMICH—Strong Communities, Healthy Children. The IMICH, held every two years, focuses on innovative clinical care models and community-based public health approaches for children and youth in First Nations, Inuit, Métis, American Indian, Alaska Native and other Indigenous communities around the world.  This meeting brings together health care providers and researchers working with children, youth and families in Indigenous communities. Participants share model programs and research, learn about prevalent health problems, and acquire practical skills for use in community settings. Opportunities to share knowledge and support one another's efforts, to network and develop partnerships are built into the program.

The Patient Experience: Improving Safety, Efficiency, and CAHPS Through Patient-Centered Care

This Institute for Healthcare Improvement (IHI) seminar will equip health care professionals with concrete tools and a roadmap for integration of new practices into existing processes. This seminar is ideal for anyone who has an impact on the patient safety experience and will teach attendees how to use practical techniques to put the emphasis on a patient's care experience—through respect, partnership, shared decision making, well-coordinated transitions, and efficiency.

Webinar: How to Use Data to Improve Care Delivery
Co-sponsored by the National Center for Medical Home Implementation and the Child and Adolescent Health Measurement Initiative
Data on children's health can be a powerful tool to educate stakeholders, inform decision makers, and motivate and track improvement of children's health care delivery in practice. Faculty for this webinar will describe how to access and use valuable national and state data sets, including those available through the Medical Home Data Portal, an online resource with state-by-state summaries and across-state comparisons on children's access to a medical home using standardized data from the National Survey of Children's Health and National Survey of Children with Special Health Care Needs. Participants will learn how to use data to improve care at their practice by hearing the experience of faculty involved in a state CHIPRA demonstration project.

This conference is part of Project Intersect: Addressing Health Disparities at the Intersection of Race, Ethnicity, and Disability. The conference is intended to bring together researchers, advocates, and policy makers in racial and ethnic disparities and disability-related disparities to learn about barriers to health care and health promotion for people with disabilities in underserved racial and ethnic groups, share your own work on health disparities at this intersection, and discuss priorities for future research and action.

Webinar: How To Incorporate Best Practices in Family Centered Care in Your Practice

This webinar, hosted by the Natinoal Center for Medical Home Implementation (NCMHI) will provide participants with a clear description of family-centered care and highlight case studies that showcase effective strategies in delivering that care through practice improvements such as greater use of health information technology, enhanced access to medical professional staff outside regular office hours, feedback mechanisms capturing patient/family experience with care, and including families on advisory boards and as staff. A recently published monograph by the NCMHI will also be highlighted, which features case studies focusing on 17 pediatric practices nominated by their peers or patients as exemplary patient- and family-centered medical homes.

26th Annual State Health Policy Conference
The National Academy for State Health Policy (NASHP) holds its annual conference during the month of October. The three-day conference is planned by state health policymakers for state health policymakers. Each year, NASHP assembles a series of carefully crafted sessions on the most pressing and emerging health care policy issues. The NASHP conference attracts hundreds of state health policy experts who are interested in information and resources to support their work. The conference offers engaging discussions with state program administrators, legislators, and other health policy experts who have the opportunity to network, share, and learn from each other.

2013 Patient Centered Primary Care Collaborative Annual Fall Meeting
The PCPCC’s Annual Fall Conference brings together the country’s most diverse and engaged groups of health care leaders and medical home experts dedicated to advancing primary care and the patient-centered medical home (PCMH). Attracting 600 attendees from across the health care sector—including health plans, large employers, professional associations, pharmaceutical firms, health systems, patient advocates, and policy makers—the conference includes panel discussions and workshops focused on key components of the medical home. These sessions focus on promoting best practices, care delivery innovations, emerging issues and public policy initiatives.

American Public Health Association (APHA) Annual Meeting
The APHA Annual Meeting & Exposition is the oldest and largest gathering of public health professionals in the world, attracting more than 13,000 national and international physicians, administrators, nurses, educators, researchers, epidemiologists, and related health specialists. The APHA meeting program addresses current and emerging health science, policy, and practice issues in an effort to prevent disease and promote health.
