Overview
For the first time this year, The American Academy of Pediatrics (AAP) offeedr
its Every Child Deserves a Medical Home Training Program as a pre-conference
to the AAP National Conference and Exhibition in Boston on October 18, 2002.
The program aims to support pediatric health care providers interested in
the well being of children with special needs in a changing health care environment.
Target
Audience
The target audience includes pediatric health professionals typically attending
the AAP National Conference and Exhibition.
Course
Objectives
Upon completion of this course, participants will be
able to:
A(1) Explain the elements of the medical
home concept as applied to their practice environment;
(2) Understand the concepts, skills, and information
necessary to successfully care for children with special
health care needs in a changing health care environment;
(3) Understand the importance, benefit, and means of
developing family-professional partnerships; (4) Be able
to identify programs and services in the community that
serve children with special health care needs; (5) Understand
the importance of transitioning children and youth to
adulthood and ways to assist this process; (6) identify
the various roles and responsibilities in providing a
medical home - in particular understand the partnership
between primary care pediatricians and subspecialists;
and (7) Be able to assess, and if necessary improve pediatric
office practices to assure that they are sensitive to
families of children and youth with special health care
needs.

Practical strategies to implement medical home into practice will be
discussed including useful CPT Codes, data management techniques and
cost-effective ways to improve office design and layout.

Screening, Surveillance and the Medical Home
Obtain a better understanding of screening and surveillance. Learn practical
strategies on integrating screening and surveillance into a clinical
practice, and about fostering effective community networking and collaboration.

Transitioning Children and Youth to Adulthood
Understand transition related issues and strategies for advancing children
and youth into adulthood.

Learn strategies to better advocate for children with special needs and
