Congenital Heart Public Health Consortium (CHPHC)The Congenital Heart Public Health Consortium (CHPHC) is a group of organizations uniting resources and efforts in public health activities. The CHPHC mission is to prevent the occurrence of congenital heart disease (CHD) and to enhance and prolong the lives of those with CHD through surveillance, population-based research, education, health promotion, advocacy, and policy development. In 2008 a meeting of experts and interested parties was organized with the goal of generating new ideas to develop a public health approach to issues surrounding congenital heart disease. Approximately 50 attendees (academicians, researchers, clinicians, non-profit organizations, federal agencies, parent groups, etc) participated in plenary and break out sessions. This meeting cultivated a common ground for key stakeholders and provided momentum for the vision and establishment of the CHPHC.
The bylaws are currently being developed and will be available soon. A logic model is a visual “snapshot” of a project (program) that communicates the intended relationship between program goals, activities, outputs and the intended outcomes. Logic models are an iterative tool useful for planning and evaluation purposes. Simply put, logic models graphically describe the theory - or logic - of how your program is supposed to work.
